چکیده

This article deals primarily with the selection criteria, production, and the nuclear, physical, and chemical properties of certain dual-purpose radionuclides, including those that are currently being used, or studied and evaluated, and those that warrant future investigations. Various scientific and practical issues related to the production and availability of these radionuclides is briefly addressed. At brookhaven national laboratory (BNL), we have developed a paradigm that involves specific individual ‘dual-purpose’ radionuclides or radionuclide pairs with emissions suitable for both imaging and therapy, and which when molecularly (selectively) targeted using appropriate carriers, would allow pre-therapy low-dose imaging plus higher-dose therapy in the same patient. We have made an attempt to sort out and organize a number of such theragnostic radionuclides and radionuclide pairs that may thus potentially bring us closer to the age-long dream of personalized medicine for performing tailored low-dose molecular imaging (SPECT/CT or PET/CT) to provide the necessary pretherapy information on biodistribution, dosimetry, the limiting or critical organ or tissue, and the maximum tolerated dose (MTD), etc., followed by performing higher-dose targeted molecular therapy in the same patient with the same radiopharmaceutical. As an example, our preclinical and clinical studies with the theragnostic radionuclide Sn-117m are covered in somewhat greater detail. A troublesome problem that remains yet to be fully resolved is the lack of availability, in sufficient quantities and at reasonable cost, of a majority of the best candidate theragnostic radionuclides in a no-carrier-added (NCA) form. In this regard, a summary description of recently developed new or modified methods at BNL for the production of five theragnostic radionuclide/radionuclide pair items, whose nuclear, physical, and chemical characteristics seem to show promise for therapeutic oncology and for treating other disorders that respond to radionuclide therapy, is provided.

The Status of Stress Myocardial Perfusion Imaging Using 99mTc Pharmaceuticals in Japan: Results from a Nationwide Survey

Objective(s): To appropriately use one-day myocardial perfusion imaging (MPI) with 99mTc radiopharmaceuticals (i.e. to avoid shine-through artifacts), injection doses need to be optimized and dose ratios between the 1st and 2nd scans should be maintained at ≥ 3. However, the current state of practice in Japan is unclear. Thus, the aim of this study was to clarify the details of MPI protocols us...

Bridging the Gap Between Research and Policy and Practice; Comment on “CIHR Health System Impact Fellows: Reflections on ‘Driving Change’ Within the Health System”

Far too often, there is a gap between research and policy and practice. Too much research is undertaken with little relevance to real life problems or its reported in ways that are obscure and impenetrable. At the same time, many policies are developed and implemented but are untouched by, or even contrary to evidence. An accompanying paper describes an innovative progr...

Vertical response of short and long span suspension bridges due to near and far fault earthquakeshttps://jeg.khu.ac.ir/files/site1/yekta_program/bb/tmpl_yekta/bb_end.gif

Many researches have been currently conducted on the effects of fault distance on structures revealing that their seismic response can differ according to their distance from the fault. Suspension bridges due to their long period and high flexibility can be more sensitive to this phenomenon, especially in vertical vibration. Since the engineers tend to use longer spans, the length factor should...

Gastrografin-based conservative approach to small bowel obstructions: too far from a daily use

As known, small bowel obstruction is one of the most common emergencies in general surgery carrying a not negligible rate of morbidity and financial expenditures as far as high social impact . Peritoneal adhesions are the leading cause of intestinal obstruction up to 74% of cases. In spite of increasing incidence worldwide, current management still presents some “blind corners”, whose aspects s...

Human Cancer Modeling: Recapitulating Tumor Heterogeneity Towards Personalized Medicine

Despite diagnostic, preventive and therapeutic advances, growing incidence of cancer and high rate of mortality among patients affected by specific cancer types indicate current clinical measures are not ideally useful in eradicating cancer. Chemoresistance and subsequent disease relapse are believed to be mainly driven by the cell-molecular heterogeneity of human tumors that necessitates perso...
